POSITION:           ESCNJ ACS Transition Coordinator

REPORTS TO:     ESCNJ ACS Executive Director

START DATE:     December 12, 2024        

 

QUALIFICATIONS:

  • Graduated from an accredited college or university with a Bachelor’s degree, or higher, in Education, Social Work, Psychology or related field, plus one (1) year of successful experience in human services
    or employment services; or

  • Graduated from an accredited college or university with an Associate’s degree, plus two (2) years of successful experience in human service; or

  • Graduated with a high school diploma or equivalent and five (5) years of experience in occupational areas similar to those being offered at the program.  A combination of college or technical school may be substituted for experience on a year for year basis; 

  • Have a clear understanding of the demands and expectations in business and industry and have 1 year of experience working with adult (18 or older) individuals with developmental disabilities

    • The experience must be the equivalent of a year of full-time documented experience working with adults (18 or older) with intellectual/developmental disabilities;

    • This experience can include paid employment, volunteer experience, and/or being a family caregiver of an adult with a developmental disability;

    • If you have previously provided employment/job coach services to a different population and some percentage of the individuals you served had developmental disabilities, you may be able to demonstrate the equivalence of a year of experience working with adults with developmental disabilities (a waiver request along with the resume detailing experience and a justification for hiring the potential Transition Coordinator may be submitted to the Division’s Assistant Director to demonstrate the experience requirement has been met); - and-

    • The ESCNJ Adult Community Services Transition Coordinator must meet all of the qualifications of Job Coach, Direct Care Staff; - and-
       

  • Must possess a valid NJ driver’s license and be insured by the state of New Jersey
  • State, Federal Criminal Background checks and Central Registry check at the time of hire; - and-

  • Successfully complete trainings required by the Division before rendering services.
     

EVALUATION: First 90 days in ISP Coordinator/Trainer position then annually
 

RESPONSABILITIES:

  • Implement person-centered best practices
  • Initiate positive communication/relationships between the ESCNJ Adult Community Services Program? and local community businesses/agencies
  • Be responsible for providing job placement services, volunteer services and community service projects to ESCNJ Adult Community Services Program participants in conjunction with ESCNJ ACS Program job coaches and local community businesses/agencies
  • Be responsible for holding periodic conferences with local community businesses/agencies, ESCNJ ACS Program staff to facilitate the learning process for ESCNJ ACS Program participants
  • Be responsible for providing job placement services, volunteer services and community service projects to ESCNJ Adult Community Services Program participants in conjunction with ESCNJ ACS Program
    job coaches and local community businesses/agencies
  • Establishes a climate that enhances the respect for local community businesses/agencies, ESCNJ ACS Program staff and participants as well as, for peer team members and colleagues
  • Be responsible for providing the ESCNJ ACS Program participants information relating to careers, work habits, attitudes and filling out appropriate forms;
  • Be responsible for assessing ESCNJ ACS Program participants with job task(s) analysis and
  • Provide on-site evaluation of performance of ESCNJ ACS Program participants;
  • Maintain progress notes, reports and documentation in ESCNJ ACS Program EHR, participant files and submit reports to ESCNJ ACS Program Executive Director;
  • Oversees and schedules community employment/volunteer services/community service programs;
  • Conducts evaluation and identification of work adjustment goals for ESCNJ ACS Program participants;
  • Prepares detailed job and task analyses, for the ESCNJ ACS Program staff to serve as the basis for developing training strategies;
  • Ensuring compliance with all qualifications, standards, and policies related to Day Habilitation as explained in The Supports Program and Community Care Program Manuals
  •  Notifying the planning team of any revisions needed and completing the ISP Revision and Notification Form
  • Assist with Medicaid and DDD audits as required
  • Conducting internal monitoring and oversight
  • Ensuring compliance with all qualifications, standards, and policies related to DDD, Medicaid and ACS
  • Performs such other duties as may be assigned.

KNOWLEDGE, SKILLS & ABILITIES:
 
  • Knowledge of Microsoft Office/Work and Excel and email required
  • Comfort in learning new tasks as needed
  • Knowledge and comfort with working in an organization serving individuals with intellectualand developmental disabilities
  • Ability to use a computer to record activities, track and monitor
  • Computer literate
  • Detail orientated
  • Experience with Electronic Health Record systems
